Totality Home Healthcare, LLC provides home healthcare services, including skilled nursing, rehabilitation services, physical therapy, occupational therapy, speech therapy, social work services, behavioral health, home health aide, and private pay nursing and aide services. Our services are available to patients referred by healthcare providers or directly by patients or their families.

To be eligible for our services, patients must meet the following requirements:

• Reside within the geographic areas we serve (primarily Milford, CT, and surrounding regions).
• Have a medical condition that qualifies them for home healthcare, as determined by a physician or healthcare provider.
• Agree to adhere to the care plan and cooperate with our healthcare professionals.

Patients and their representatives are expected to:

• Provide accurate and complete health information to facilitate care.
• Comply with the agreed-upon care plan and any medical instructions provided by our healthcare team.
• Treat our staff with respect and ensure a safe environment for them to perform services.
• Notify us promptly of any changes in condition, address, or contact information.
• Ensure that any payments, whether through insurance or private pay, are made in a timely manner.

Our services are covered by Medicare, Medicaid, and private insurance, where applicable. For services not covered by insurance or government programs, we offer private pay options. By agreeing to our services, you agree to:

• Provide accurate and up-to-date insurance information.
• Be responsible for any co-payments, deductibles, or out-of-pocket expenses not covered by insurance.
• Pay any private pay services in accordance with the fee schedule provided during service initiation.

Your privacy is important to us. All personal health information (PHI) collected during the course of providing care will be handled in accordance with our Privacy Policy and the Health Insurance Portability and Accountability Act (HIPAA). We will not disclose your information to third parties without your consent, except as required by law or for treatment, payment, and healthcare operations.

This includes, but is not limited to, any indirect, consequential, or punitive damages.

You agree to indemnify and hold harmless Totality Home Healthcare, LLC, its employees, and agents from and against any claims, liabilities, damages, or expenses (including reasonable attorney’s fees) arising out of your use of our services or your breach of these Terms.

These Terms and Conditions are governed by and construed in accordance with the laws of the State of Connecticut. Any disputes arising under these Terms will be subject to the exclusive jurisdiction of the state or federal courts located in Connecticut.
