Patient Responsibilities

Patients and their representatives are expected to:

  • Comply with the agreed-upon care plan and any medical instructions provided by our healthcare team.
  • Treat our staff with respect and ensure a safe environment for them to perform services.
  • Notify us promptly of any changes in condition, address, or contact information.

Payment and Billing

Our services are covered by Medicare, Medicaid, and private insurance, where applicable. For services not covered by insurance or government programs, we offer private pay options. By agreeing to our services, you agree to:

  • Be responsible for any co-payments, deductibles, or out-of-pocket expenses not covered by insurance.
  • Pay any private pay services in accordance with the fee schedule provided during service initiation.
